For Minors
If you want to use the patient portal for a child who's 11 or younger, you need to complete a FollowMyHealth Parental/Guardian Access Application.
Once you fill out the application, it can take 3-5 days for you to be approved to access your child’s health records.
Note: If the parent or child is between 12 and 17 years old, we are unable to make medical records accessible via FollowMyHealth Portal due to state regulations.
With the FollowMyHealth patient portal, you can:
- Communicate with your provider via secure messages
- View your Test and Lab Results
- Request Rx Refills
- Request an Appointment
- Pay Your Bill Online
FollowMyHealth Alerts
Once enrolled, you will receive alerts when you have new health information available in your FollowMyHealth portal. It's important to know this information may be available before your provider has a chance to review or call you. Once reviewed, your provider may add comments into your electronic health record.

From an app on your smart phone or device
If you want to use your smart phone, download from the Apple App Store or Google Play to install the app on your device.
