For Minors
If you want to use the patient portal for a child who's 11 or younger, you need to complete a FollowMyHealth Parental/Guardian Access Application.
Once you fill out the application, it can take 3-5 days for you to be approved to access your child’s health records.
Note: If the parent or child is between 12 and 17 years old, we are unable to make medical records accessible via FollowMyHealth Portal due to state regulations.

Important Note on FollowMyHealth Alerts
Once enrolled, you'll receive alerts when new health information is available in your portal. Please be aware that this information may be accessible before your provider has reviewed it. After reviewing, your provider may leave comments in your electronic health record.

FollowMyHealth Patient Portal FAQ
The portal can be used for simple requests such as:
If you have a quick medical question (e.g., “Can I take this allergy med with my new prescription?”).
To request a routine prescription refill.
To follow up on a recent visit (e.g., “The new medication is working well!”).
To request or reschedule a non-urgent appointment.
Please do not use our portal for the following items—if it's an emergency, call 911.
If you have emergency symptoms (e.g., chest pain, trouble breathing, worsening illness), call our office or go to the ER.
If you have a new or complex issue with multiple symptoms, schedule an appointment instead of messaging.
If you need a same-day response—portal messages are reviewed within 2 business days.

Accept terms and connect to “The Iowa Clinic Corporate Office.”
Your account will be linked within 3 business days.
Call 515.875.9250 during business hours for a recovery email.
If you don’t receive a password reset email, check your junk/spam folder or call us.
Messages are reviewed within 2 business days.
If your concern is urgent, call our office, do not send urgent messages in the portal.
If your child is 0-11 years old, you can request parental access.
Due to state regulations, we cannot offer portal access for ages 12-17.
