DOT Physicals
If a DOT physical is necessary for your work place, we can help. These physicals follow strict rules and must be conducted by a FMCSA certified provider or physician.
When coming in for a DOT physical you can expect to:
- Provide full health history
- Provide complete list of medications (including dosages)
- Bring most recent Hemoglobin A1C lab results if diabetic. Also bring in blood sugar logs.
- Bring (if applicable) all vision and hearing devices (ie: glasses and hearing aids)
- Letter from any specialist you may see (cardiologist, neurologist, pulmonologist, etc.).
During the exam your provider will conduct vision, hearing, blood pressure and urine analysis exams. Additionally, a physical exam will be conducted.
To get your DOT physical scheduled, call our Primary Care front desk at 515.875.9000 to see if there is a certified DOT physical provider near you.
